We are committed to giving every child in Scotland the best start in life. Women affected by substance use should be able to get the help they need, free from judgement and stigma, throughout the perinatal period.

All Health Boards have guidance for health professionals to ensure pregnant women with substance use issues receive the support they need to reduce the risk of adverse effects on their child and we have also asked Health Boards to prioritise continuity of midwifery care for women affected by substance use.

In March 2024, we brought together an expert working group who are developing a good practice guide to support local areas to do more to meet the needs of women affected by substance use and their infants during pregnancy and the first postnatal year.

The third sector plays a key role for families in providing access to perinatal mental health support, and the Perinatal and Infant Mental Health Fund offers funding to third sector organisations who provide support services to women, including those affected by substance use, during this period. Many of these services provide emotional and practical support to families throughout pregnancy and create a trauma informed environment where parents are supported to give their babies the best possible start in life. We are investing a further £1.5 million over 18 months through this fund, which will continue until March 2026.

The Scottish Government recognises the importance of highlighting the link between alcohol and non-communicable disease. Evidence is clear that alcohol consumption increases the risk of developing non-communicable diseases, including some cancers.

A refreshed cross-government and cross-sectoral approach to population health - focused on prevention - is being progressed. Increasing healthy life expectancy and reducing health inequalities across Scotland remains a clear ambition for this Government. It is clear the reform of health and care services alone will not be enough to stem the current tide of population health decline in Scotland.

The Population Health Framework is being developed jointly by the Scottish Government & COSLA and in collaboration with Public Health Scotland, Directors of Public Health and key local, regional and national partners. The Scottish Government will set out key actions to reduce alcohol harm following the Population Health Framework, including the role of public health messaging and awareness.

The Scottish Government remains committed to improved health messaging on alcohol in Scotland, including on labels and packaging. The Scottish Government will continue to engage constructively with the UK Government, along with our colleagues in Wales and Northern Ireland devolved administrations, in relation to progressing work on alcohol labelling.

In July 2024, the Scottish Government published the first annual progress report on the Cancer Action Plan 2023-26. The next progress report of the Cancer Action Plan is due to be published in the summer 2025. Additionally, the Population Health Framework will consider key actions to improve population health, including on alcohol harm prevention, in line with World Health Organisation guidance on reducing the affordability, attractiveness and availability of alcohol.

Information is not held centrally on the most common way to consume illicit drugs.

In 2023, 14% of all adults had used any of the drugs listed in the Scottish Health Survey in the previous 12 months. The drug most commonly used in the 12 months prior to being interviewed was cannabis (9%) (The Scottish Health Survey 2023 - volume 1: main report - gov.scot). Cannabis is typically smoked.

The Drug and Alcohol Information System (DAISy) captures information on people seeking specialist drug treatment. Public Health Scotland publish statistics annually presenting an overview of initial assessments for specialist drug and alcohol treatment (Drug and alcohol information system (DAISy) overview of initial assessments for specialist drug and alcohol treatment 2023/24 - Drug and alcohol information system (DAISy) - Publications - Public Health Scotland). This includes an analysis of the route of both cocaine and heroin use.

Public Health Scotland also publish information on the provision of injecting equipment (Injecting equipment provision in Scotland - 2022 to 2023 - Injecting equipment provision in Scotland - Publications - Public Health Scotland) along with information from their Needle Exchange Surveillance Initiative (NESI) which aims to measure the uptake of harm reduction services among people who inject drugs in Scotland (Needle Exchange Surveillance Initiative (NESI) 13 August 2024 - Needle Exchange Surveillance Initiative (NESI) - Publications - Public Health Scotland).

Public Health Scotland publish information on drug related hospital admissions. Data from 1996-97 to 2022-23 is available at https://publichealthscotland.scot/publications/drug-related-hospital-statistics/drug-related-hospital-statistics-scotland-2022-to-2023/. The release of data covering 2023-24 is expected in the spring. The accompanying Data Explorer Dashboard provides a more comprehensive breakdown of the statistics, including by age.

The Scottish Government continues to work closely with the UK Government to disrupt organised crime groups involved in the illicit drugs trade and the misery it causes to individuals and communities. As I explained in my answer to Parliamentary Question S6W-32543 on 10 January 2025, we had previously promoted a Legislative Consent Memorandum in the Scottish Parliament in relation to the provisions in the previous UK Government’s Criminal Justice Bill creating a new offence of importing, making, adapting, supplying or offering to supply a relevant article for use in serious crime. This offence would have included pill presses in addition to other articles but the legislation fell due to the change of administration.

My officials are continuing to engage with the Home Office to explore the potential for this offence, and others that disrupt aspects of the illicit drug trade such as cuckooing, to be taken forward in one of the bills that will be introduced in early 2025. In the event that such legislation is brought forward, we will give careful consideration to the possibility of extending such measures to Scotland.

The criminal exploitation of young people through involvement in the illicit drug trade is a significant concern for the Scottish Government. Partners on the Serious Organised Crime Taskforce (SOCT), which I chair, continue to use every means at their disposal to disrupt serious organised crime, including illicit drugs, and to raise awareness of the impact it has on individuals, including young people, and our communities.

The Scottish Government published 'Practitioner Guidance on Criminal Exploitation' in June 2023 on behalf of the SOCT. The guidance aims to maximise Scotland’s coordinated and joined up response to all exploited individuals, making it easier for practitioners across Scotland to support children and vulnerable adults who have been victims of exploitation. The SOCT also held a criminal exploitation conference in June 2024 to publicise the guidance.

The latest SOCT Progress Report, published in December, details some of the work over the past year, including projects such as the Daniel Spargo-Mabbs Foundation’s very successful Drugs Education Pilot in Grampian schools and the “Fearless” drugs campaign which was launched with young people at a school in Edinburgh.

We had previously promoted a Legislative Consent Memorandum in the Scottish Parliament in relation to the provisions in the previous UK Government’s Criminal Justice Bill creating an offence for importing, making, adapting, supplying or offering to supply a relevant article for use in serious crime. This offence would have included vehicle concealments as an article of serious crime but the legislation fell due to the change of administration.

My officials are continuing to engage with the Home Office on its legislative proposals. I hope that this offence will be taken forward in one of the bills that will be introduced in early 2025.

Police Scotland will be also be kept informed as soon as any legislation is developed.

The Scottish Government or Public Health Scotland do not currently publish information on the outcome or discharge status of admissions and are therefore unable to provide information on hospitalisations specifically attributed to non-fatal drug misuse.

The Scottish Government does not have responsibility for addressing the improper disposal of used hypodermic needles. We expect that NHS Boards, drug and alcohol treatment services, and third sector organisations will have their own plans in place to address this issue.

Injecting equipment provision (IEP) is a key harm reduction intervention. Public Health Scotland publish information on IEP in Scotland, including the number of outlets, attendances, and the types of injecting equipment distributed to people who used illicit drugs.
